Your medical needs remain our top priority. Therefore, we recommend that you should establish with a new provider as soon as possible.

Meanwhile, your medical records remain with the clinic to ensure a seamless transition. Please note that we cannot share your medical information without your written consent.  A signed Medical Record Release Form can be emailed to irvingmedicalclinic@gmail.com or faxed to 972-887-3349.
